Add the application form to your store

Put B2B Desk's wholesale application form on its own page of your Shopify store, publish your form fields to it, and link buyers to it.

Updated 3 min read

The application form is a theme block called Wholesale application. Buyers fill it in on your store without needing a customer account, and each submission shows up in B2B Desk under Applications. Nothing is created in Shopify until you approve it.

Most stores put the form on a dedicated page, such as /pages/wholesale. That’s also where the wholesale bar sends visitors by default.

Create the wholesale page

  1. In your Shopify admin, go to Online Store › Pages and click Add page.
  2. Title it Wholesale. Shopify gives it the address /pages/wholesale. Add any text you want above the form, then click Save.

Give the page its own template

The theme editor adds blocks to a template, and every page using that template shows them. Your default page template is shared by pages like About and Contact, so create a separate one for the form.

  1. Go to Online Store › Themes and click Customize on your live theme.
  2. In the page selector at the top of the theme editor, choose Pages, then Create template.
  3. Name it wholesale, base it on Default page, and click Create template.

Add the Wholesale application block

  1. With the wholesale template open, click Add section and choose Wholesale application from the Apps tab.
  2. Set the block’s Heading (default “Apply for a wholesale account”) and an optional Introduction.
  3. Click Save.
  4. Back in your Shopify admin, open the Wholesale page, set its theme template to wholesale, and save.

B2B Desk’s Add form to a page button (on Home) and Add form to your store (on an empty Applications page) open the theme editor with the block already added, but on your default page template. Use them when you want the form on every default page, or switch to your wholesale template before saving.

Publish your form fields to the store

After that first save, every later save updates the form on your store. The hosted application page always uses the latest saved form.

What applicants see

  • Required fields are marked with an asterisk, with the note “Fields marked * are required.”
  • Fields are checked as the applicant leaves each one, and errors show next to the field. On submit, problems are listed with “Please fix the highlighted fields.”
  • After a successful submit the form is replaced by “Thank you, we received your application” and “We will review it and email you. A confirmation is on its way to your inbox.”

The form also works when a visitor’s browser blocks JavaScript. The page reloads after submitting and shows the same thank-you message, or “Your application was not sent” if something was wrong.

Make the page easy to find:

  • Turn on the wholesale bar. Visitors see “Buying for a business?” with a link to /pages/wholesale.
  • Add the page to your store’s menu or footer under Online Store › Navigation.

To check the block is live, see the Storefront card on B2B Desk’s Home page. It should show Active with “On Wholesale page”. See Check that your storefront blocks are live.
