Choose the fields on your application form

Add, remove, reorder and edit fields in B2B Desk's form builder, including the fields Shopify needs, standard fields and up to 20 custom questions.

Updated 3 min read

You build the application form in Settings › Application form. The Fields view lists every field in order, and a live Preview beside it updates as you edit. Switch the preview between Desktop and Phone, and click a field in the preview to edit it.

The fields Shopify needs

Three fields are always on the form and always required, because approving an application uses them to create the company in Shopify:

Field Why it’s locked
Company name “Shopify names the company with it.”
Email “The company contact signs in to your store with it.”
Billing address “It becomes the address of the company location.” Street, city and country are required.

If one of them is missing, the form can’t be saved, and B2B Desk offers Add company name, Add email or Add billing address to put it back.

Standard fields

B2B Desk knows where standard fields go in Shopify, and uses some in duplicate checks and auto-approve rules. Each can be added once.

Field What it’s for
Website Saved on the company in Shopify
Tax ID / VAT number Spots duplicate companies and can drive auto-approve rules
First name, Last name The contact’s name, used in emails
Phone Saved on the contact and the location
Shipping address Lets buyers ship somewhere other than the billing address. Buyers see “Shipping address is the same as billing” ticked and can untick it.
Expected monthly volume A dropdown of your ranges. Rules use the lowest amount in each range.
How did you hear about us? A short text answer
Resale certificate A PDF, JPG or PNG upload, saved to your Files
Agreement A statement buyers must tick to apply, such as “I agree to the wholesale terms.” Always required, and always last.

Your own questions

Click Add field and pick a type under Your own question. Answers show on the application and are saved on the company in Shopify when you approve.

Type Use it for
Short answer One line, like a registration number
Paragraph Several lines for longer answers
Number A number, with an optional range
Email An email address, like for invoices
Phone number A phone number
Website link A website or social profile
Date A date, like when the business opened
Dropdown One choice from a dropdown list
Multiple choice One choice, every option shown
Checkboxes Any number of choices from a list
Checkbox One box to tick
File upload A PDF, JPG or PNG up to 10 MB

New questions start with an example label you can change, such as “Business registration number” for a short answer. To group fields, add a Section heading from Layout, with an optional description.

Edit a field

Click Edit on a field’s row. Changes stay in the editor until you click Done.

  • Label: up to 120 characters.
  • Help text: shown under the label, up to 500 characters. Use it to say why you ask.
  • Placeholder: example text inside the empty field. Don’t put instructions here, because they disappear as buyers type.
  • Choices: one per line, up to 50.
  • Required: buyers must answer to apply.
  • Width: Full width or Half width. Half-width fields sit side by side on wider screens.
  • Validation: rules for what counts as a valid answer. See Set validation rules.

Custom questions also have an Answer key, which auto-approve rules use to read the answer. It follows the label until you change it. If you change the key on a saved field, answers you already received keep the old key and rules that check it stop matching.

Reorder, copy and remove

  • Drag a row to move it, or use the … menu for Move up and Move down.
  • Duplicate in the … menu copies a custom question below the original.
  • Remove in the … menu deletes the field. Click Undo in the message that appears to put it back.

Save the form

Shopify’s save bar appears once you change something. Saving checks every field first. If something needs fixing, a banner such as “Fix 2 problems to save the form” links to each field. A successful save shows “Application form saved” and updates the form on your store and the hosted page.

Limit Message
20 custom questions “Use up to 20 custom fields.”
10 section headings “Use up to 10 section headings.”
11 file uploads “Use up to 11 file uploads.”
50 choices per field “Use up to 50 choices, each under 120 characters.”
